public healthFREE Newborn Home Visits by a Grand County Public Health Nurse
Every parent of a new baby -- no matter how experienced or well informed -- benefits from a home visit from a registered nurse. Ideally, every newborn should have an evaluation 48-72 hours after discharge from the hospital to include weight, formal breastfeeding/feeding evaluation, oxygenation check at our altitude and much more.
A home visit during the postpartum period will serve to decrease other possible complications for example; newborn jaundice, infections, postpartum depression, inadequate weight gain, failure to thrive and developmental concerns.
A newborn home visit also connects parents to community resources.

REACH OUT AND READ (ROR)
Reach Out and Read (ROR) programs make early literacy a standard part of pediatric primary care. Following the ROR model, pediatricians, family physicians and nurses advise parents that reading aloud is the most important thing they can do to help their children love books and to start school ready to learn.
Children who live in print-rich environments and who are read to during the first years of life are more likely to learn to read on schedule.
Grand County Public Health nurses encourage parents to read aloud to their young children, and offer age-appropriate tips and encouragement. Nurses give every child between the ages of six months and five years new, developmentally appropriate children’s book to keep.
Statewide, Reach Out and Read serves 78,000 kids and distributes 128,000 new books annually.

Presumptive Eligibility site for:
Medicaid: insurance program for children from low income families.
CHP+ (Child Health Plan Plus). A comprehensive insurance program for Colorado children who do not qualify for Medicaid.

CALL TO SEE IF YOU QUALIFY. We will help you apply, obtain the necessary documentation, and give you “Presumptive Eligibility” which allows your child to see a doctor right away.

Referrals to EPSDT
Early and Periodic Screening Diagnosis and Treatment Program
• Assists Medicaid clients (up to age 21) with medical, dental and vision referrals.

WIC - Nutrition Education
Women, Infants and Children Program
• Nutritious foods and education for income eligible women who are pregnant or breastfeeding and children ages 0-5.
• Lactation Management Consultant: breastfeeding information and support.

Health Care Program for Children with Special Needs
• Assistance in finding resources to address medical, developmental, emotional,and physical disorders. There is no income qualification and no medical diagnosis required.
